Axis Community Health is a nonprofit healthcare provider based in California. On August 26, 2024, Axis Community Health discovered unusual activity within their system, and determined that an unauthorized party gained access to their computer systems between July 9, 2024 and September 4, 2024. Axis Community Health confirmed the exposed data may include Social Security numbers and driver’s license numbers.
