Community Catalyst, Inc. is a healthcare equity  advocacy group located in Boston, Massachusetts. Recently, Community Catalyst sent notice letters out alerting recipients to a breach which had compromised their private data. The extent of the breach has not been made public, but the individual notice letters should contain more details.
