Conifer Value-Based Care is a revenue cycle management solutions company based in Encino, California which serves physicians and healthcare practices. In August of 2025, Conifer’s computer network was infiltrated by an unauthorized party who gained access via an employee’s email account. Customer notice letters were mailed to affected individuals on December 18, 2025. These letters should contain specifics regarding the exact data which was exposed for each recipient.
