Fyzical Acquisition Holdings, LLC owns a physical therapy chain called FYZICAL Therapy & Balance Centers with their headquarters in Sarasota, Florida and other locations in Illinois and Texas. On December 9, 2024, Fyzical discovered a data breach to their computer network. Affected individuals were not sent notice letters until more than a year later — on December 19, 2025. The notice letters should provide specifics regarding which data was breached, but it might include any combination of names, Social Security numbers, birth dates, financial account information, health insurance or medical data.
