PROVAIL is a Washington based company that provides services to adults with intellectual and developmental disabilities and traumatic brain injuries. Between May and June 2023, PROVAIL experienced a breach by an unauthorized party, who viewed or accessed sensitive information. On August 8, 2025, PROVAIL reported the breach to the U.S. Department of Health and Human Services, and on January 16, 2026, the breach was disclosed to the Washington Attorney General. PROVAIL confirmed the exposed data may include names, Social Security numbers, addresses, driver’s license numbers, medications, and other sensitive information. Notice letters have been sent out to affected parties.
