Between November 11, 2024 and May 14, 2025, TriCity Family Services discovered unauthorized access within their systems. The incident was officially reported to the U.S. Department of Health and Human Services on December 8, 2025, and it was confirmed that the exposed data may include names, dates of birth, requested treatments, provider locations, and more.
