Community Health Northwest Florida is a non-profit, equal opportunity health center based in Florida. On December 24, 2024, Community Health Northwest Florida discovered a breach by an unauthorized party, who viewed or accessed sensitive information on the company’s computer system. On January 19, 2026, Community Health Northwest Florida confirmed the exposed data may include names, Social Security numbers, driver’s license numbers, financial account numbers, and other sensitive personal information. On January 27, 2026, Community Health Northwest Florida officially reported the breach to the Vermont Attorney General. Notice letters are being sent out to affected parties.
