Consulate Health Care Data Breach

Consulate Health Care provides senior healthcare services, mainly in the form of post-acute care or short-term rehabilitation after a hospital stay.  They have locations in Pennsylvania, Virginia, North Carolina, Mississippi, and Louisiana. Consulate Health Care uses the services of a vendor called Synergy Healthcare Services. Synergy Healthcare Services recently announced a security breach to their computer system which exposed the sensitive information of 58,034 individuals.  

The information accessed by the hackers may include names, contact information, dates of birth, signatures, insurance information, driver’s license numbers, Social Security numbers, financial account information and protected health information.
