Episource LLC is a risk-adjustment company in Gardena, California which serves healthcare providers. Recently, Episource announced that a hacker had gained entry into their computer files in February of 2025 and accessed personal information stored there. On April 23, 2025, Episource began notifying individuals whose data was compromised during the breach. The information exposed varies by individual but may include names, addresses, emails, phone numbers, Social Security numbers, dates of birth, medical treatment information, and health insurance information.
