Gateway Community Services is an addiction treatment and mental health center in Jacksonville, Florida. They handle confidential patient information, and this week they sent out notice letters regarding a data breach which compromised patient files. 34,498 Gateway Community patients were affected by the breach. Their protected health information was accessed by hackers, possibly including names, addresses, Social Security numbers, birth dates, medical information, health insurance information and more.
