Independence Living Centers uses the services of a vendor called Synergy Healthcare Services. Synergy Healthcare Services recently announced a security breach to their computer system which exposed the sensitive information of 58,034 individuals.

The information accessed by the hackers may include Independence Living Centers patient names, contact information, dates of birth, signatures, insurance information, driver’s license numbers, Social Security numbers, financial account information and protected health information.
