ParkTree Community Health Center Data Breach

Pomona Community Health Center operates in Pomona, California under the name ParkTree Community Health Center.  On August 21, 2024, they filed a notice of data breach with the Attorney General of California. This notice described a breach to the ParkTree Community Health Center computer system which exposed private data stored there, including patients’ names, dates of birth, health plan information, medical record numbers, medical information and possibly more.
