Revere Health is a Utah medical provider network which recently disclosed to the U.S. Department of Health and Human Services that they had experienced a data breach of their IT systems.  During the breach, sensitive information was accessed by an unauthorized party. This may have included names, birth dates, Social Security numbers, driver’s licenses, contact information, health insurance information, medical and financial information.
