Sapphire Community Health provides primary care healthcare services to the Hamilton, Montana area. Recently, Sapphire Community Health announced that sensitive files were exposed during a data breach of their electronic health records vendor (Cerner Corporation). This incident occurred in January of 2025, and may have compromised names, Social Security numbers, birth dates, medical record data and more.
