Valley Oaks Health Data Breach

Valley Oaks Health is a healthcare provider based in Lafayette, Indiana which also provides mental health and addiction treatment services to the surrounding counties.  On March 18, Valley Oaks Health sent out letters to 50,352 affected individuals notifying them of a cyberattack which exposed their private information.   The personal identifying information stolen may include the individuals’ names, Social Security numbers, health insurance information, dates of birth, and medical treatment information.
