Primary Health-SMMPP is based in Arizona and distributes rapid Covid test kits to schools and organizations in other states as well. In February 2025, Primary Health disclosed a data breach to their computer system which affected the personal information belonging to 67,567 individuals. The information accessed by hackers may include names, dates of birth and Social Security numbers.
